As DENT FORUM ORAL AND DENTAL HEALTH SERVICES TRADE LIMITED COMPANY (“Company”), we exercise the utmost care to ensure that your personal data is processed and retained in compliance with the Turkish Personal Data Protection Law No. 6698 (“KVKK”). Accordingly, pursuant to Article 10 of the relevant Law, we process your personal data as described below and within the limits prescribed by applicable legislation. Through this Privacy Notice, we aim to fulfill our obligation under Article 10 of the Turkish Personal Data Protection Law No. 6698 to inform you about the purposes and legal grounds for processing your personal data and your rights regarding such processing.

1. Data Controller and Representative

Pursuant to the Turkish Personal Data Protection Law No. 6698 (“KVKK”), your personal data may be processed within the scope described below by DENT FORUM ORAL AND DENTAL HEALTH SERVICES TRADE LIMITED COMPANY, Tax No. 2921018203, located at Narlı Mahallesi Mithatpaşa Caddesi No: 262/1B, Narlıdere / İzmir, Türkiye, acting as the data controller.

2. Purposes for Which Personal Data Is Processed

You are using our Company’s website. Through the “Contact” form available on our website, we aim to facilitate and accelerate the process of receiving appointment requests from our clients/patients and providing information regarding applications and relevant conditions.

The personal data processed during the period in which we contact and respond to our clients/patients is considered as part of an integrated processing activity, and you are therefore informed through this single Privacy Notice.

Your personal data may be processed by our Company within the scope of the personal data processing conditions specified under Article 5 of the Turkish Personal Data Protection Law No. 6698 and for the following purposes:

  1. Identity Data (Full Name): For the purposes of conducting and supervising business activities and responding to messages submitted through the Contact Form.
  2. Contact Data (Telephone Number, Email Address): For the purposes of conducting and supervising business activities and responding to messages submitted through the Contact Form.

In order to improve the operation and performance of our website, cookie information, details of which are provided in our Cookie and Privacy Policy, may also be collected. Within this scope, your IP address may also be processed by our Company.

3. To Whom and for What Purposes Processed Personal Data May Be Transferred

Your collected personal data may be transferred to private-law persons in accordance with the law, limited to the purposes specified above and within the framework of the personal data processing conditions and purposes set forth under Article 8 of the Turkish Personal Data Protection Law No. 6698.

Furthermore, within the scope of Article 9 of the relevant Law, your processed personal data is not transferred to any natural or legal person abroad.

4. Method and Legal Basis for Processing and Transferring Personal Data

Your personal data is collected electronically and directly from you by our relevant department for processing in connection with the Company’s activities and for the purposes and reasons specified under Sections 2 and 3 of this Privacy Notice.

All personal data specified under Section 2 of this Privacy Notice is processed by automated means based on the legal ground set forth under Article 5 of the Law:

“Processing of personal data is necessary for the legitimate interests of the data controller, provided that such processing does not violate the fundamental rights and freedoms of the data subject.”

5. Rights of the Personal Data Subject Under Article 11 of the Turkish Personal Data Protection Law No. 6698

As a personal data subject, if you submit your requests regarding your rights to us through the methods described below, your request will be concluded free of charge as soon as possible and no later than thirty (30) days, depending on the nature of the request, pursuant to Article 13(2) of the Turkish Personal Data Protection Law No. 6698.

However, if processing the request requires an additional cost, a fee may be charged in accordance with the tariff determined by the Turkish Personal Data Protection Board.

Within this scope, personal data subjects have the following rights:

  • To learn whether their personal data is being processed;
  • To request information if their personal data has been processed;
  • To learn the purpose of processing their personal data and whether such data is being used in accordance with that purpose;
  • To know the third parties to whom their personal data has been transferred within Türkiye or abroad;
  • To request the correction of personal data if it has been processed incompletely or inaccurately and to request that the actions taken in this regard be communicated to third parties to whom the personal data has been transferred;
  • To request the deletion or destruction of personal data where the reasons requiring its processing no longer exist, even though such data has been processed in accordance with the Turkish Personal Data Protection Law No. 6698 and other applicable legislation, and to request that the actions taken in this regard be communicated to third parties to whom the personal data has been transferred;
  • To object to any adverse result arising against the data subject as a result of the analysis of processed personal data exclusively through automated systems;
  • To request compensation for damages suffered as a result of the unlawful processing of personal data.

Pursuant to Article 13(1) of the Turkish Personal Data Protection Law No. 6698, you may submit your request regarding the exercise of the rights specified above to our clinic in writing.

If additional application methods are subsequently determined and announced, Dent Forum Oral and Dental Health Services Trade Limited Company will also announce how applications may be submitted through such methods.

Accordingly, the channels and procedures for submitting written applications to our clinic within the scope of Article 11 of the Turkish Personal Data Protection Law No. 6698 are described below.

In order to exercise the rights specified above, you may submit your request containing the information necessary to verify your identity and explanations regarding the right or rights you wish to exercise under Article 11 of the Turkish Personal Data Protection Law No. 6698, together with documents verifying your identity, by hand to:
